a. Registration Fee for issue of health card is 2500 INR.
b. Health card will cover 6 persons - self, spouse, two children, father & mother. All the services available to card.
holder will be available to all the persons named in the card.
c. OPD/ Consultancy for two times in a year will be free for all the persons name in the card in any of the
empaneled hospital.
For treatment in any empaneled hospital discount shall be provided as under:
Hospitals will provide the following discount to the card holders.

f. Before discharge of the patient information has to be given to the foundation on toll free no by the patient.
g. At the time of admission in hospital or availing OPD services health card has to be produced to the hospital for
entering the details of services availed.
h. Online consultancy regarding any health issue will be free by 24*7 to all the card holders.
i. Foundation will update all the card holders regarding all the health services available at free/ discounted rate.

assistance & guidance on taking such card by card holder.
k. Ambulance services on call shall also be provided but it will be on chargeable basis. Cardholder can avail this service by making a call on the toll free no of foundation.

m. Home collection facility for sample for pathology test
will also be available without any extra cost. Further
30% discount on MRP for pathology test will be
provided.
n. Nurses for homecare on chargeable basis will also
be provided.
